By submitting your information or accepting leads from CareConnect, you agree to these Terms.
CareConnect aims to provide high-quality, relevant patient leads, but does not guarantee outcomes or conversion rates.
1. Service Overview
CareConnect Platform connects clinics with potential patients seeking physiotherapy services.
CareConnect Platform operates as a lead generation and patient connection service. We do not provide medical services or guarantee patient outcomes.
2. Lead Delivery
We provide patient inquiries (“leads”) based on user submissions through our platform.
A lead is considered delivered when:
• Patient information is successfully shared with your clinic
• The inquiry matches your service and location criteria
3. Payment Terms
Clinics agree to pay a fixed fee per confirmed appointment generated through CareConnect.
• Fees apply only to confirmed appointments
• No fees are charged for unresponsive or invalid leads
• CareConnect will invoice clinics based on confirmed appointments. Payment is due within the agreed billing period. Failure to pay may result in suspension or termination of services.
• A confirmed appointment refers to a patient who has successfully booked and attended an appointment with the clinic.
4. Clinic Responsibilities
By using CareConnect, you agree to:
• Respond promptly to all booking requests
• Provide accurate service information
• Maintain professional communication with patients
• Honor confirmed appointments
Failure to respond or repeated poor handling of leads may result in removal from the platform.
5. No Guarantee of Volume
CareConnect does not guarantee a specific number of leads or appointments.
Lead volume may vary based on market conditions, demand, and campaign performance.
6. Limitation of Liability
CareConnect Platform is not responsible for:
• Patient no-shows or cancellations
• Treatment outcomes or patient satisfaction
• Any disputes between clinics and patients
You agree to use the service at your own discretion.
7. Termination
We reserve the right to suspend or terminate access to the platform at any time if these Terms are violated.
8. Modifications
These Terms may be updated from time to time. Continued use of the platform constitutes acceptance of any changes.
9. Non-Exclusivity
CareConnect Platform operates as a non-exclusive service. Clinics are free to obtain patients from other sources, and CareConnect may work with multiple clinics in the same area.
10. Disputes
Any disputes regarding leads or appointments must be reported within 3 days of delivery. After this period, the lead will be considered valid.
11. Governing Law
These Terms shall be governed by the laws of Ontario, Canada.
12. Updates to These Terms
CareConnect Platform may update these Terms of Service from time to time. Any changes will be posted on this page, and continued use of the platform constitutes acceptance of the updated Terms.
